Role Summary We are seeking compassionate, reliable, and professional caregivers (DSPs) to provide one-to-one, community-based support to individuals receiving Customized Community Supports (CCS-I) under New Mexico's Developmental Disabilities (DD) Waiver. You will be matched with one person and spend your shifts out in the community with them, delivering personalized support that promotes independence, skill-building, and real inclusion, helping them live their preferred life. Key Responsibilities Community Integration: Support your client to explore their community, build relationships, join local clubs and activities, and pursue volunteer or employment opportunities under the "Employment First" principle. This service happens out in the community, not in a facility. Goal Implementation: Carry out individual-specific Action Plans, Desired Outcomes, and Teaching and Support Strategies (TSS) outlined in the Individual Service Plan (ISP). Activity Planning: Help build and follow your client's individualized activity schedule, outlining planned activities by day, week, and month. Skill Building: Support skills like communication, money management, self-advocacy, and choice-making, then help your client apply them in real settings like shopping, banking, and volunteering. Health & Safety Monitoring: Watch for and immediately report any changes in physical, nutritional, or behavioral condition to the Agency Supervisor and Registered Nurse (RN). Electronic Documentation: Log your shifts in real time through the Lift mobile app, strictly adhering to HIPAA regulations. Medication Assistance: If your client needs medication support during community hours, assist under the clinical supervision and delegation of our Agency Nurse as planned in the ISP. Safety & Advocacy: Prevent and immediately report any suspicions of Abuse, Neglect, or Exploitation (ANE) in accordance with New Mexico DHI regulations. Training All required training is provided and will be completed as part of onboarding. This includes NM core training (ANE Awareness, HIPAA, Keys to Health), First Aid and CPR certification, and individual-specific training for the person you'll support. Training must be completed within 30 calendar days of hire and prior to working alone. You'll also have a named supervisor who meets with you in the field every month, and a nurse you can call. Requirements & Qualifications A compassionate, respectful, and person-centered mindset. Must be 18 or older with a high school diploma or GED. Must pass a Caregiver Criminal History Screening Program (CCHSP) background check and Employee Abuse Registry (EAR) check. Ability to complete all required training within 30 calendar days of hire and prior to working alone. Must have reliable smartphone and internet access to use the Lift app for shift documentation. Valid New Mexico Driver's License, proof of valid auto insurance, reliable vehicle, and a clean driving record (no DUIs or at-fault accidents within the previous two years). Transportation is part of this role since services happen in the community. Experience with specialized care (such as autism, developmental disabilities, or behavioral support) is a plus, but not required. We provide comprehensive training!
