Habitat Health's PACE program empowers older adults in Sacramento and South Los Angeles to maintain their independence and keep living in their homes by providing health insurance coverage, medical care, social services, physical therapy, occupational therapy, prescriptions, transportation, day center social activities, and meals all under one roof at our Sacramento and Compton centers. We also provide in-home care for seniors who need assistance with activities of daily living.
